Depression: symptoms, causes and treatment in Pakistan
Depression is not weakness of faith or character, and it rarely lifts by being told to be grateful. It is a treatable condition affecting mood, energy, sleep, appetite and the ability to think clearly and it responds to therapy in the large majority of cases.
Signs of depression
Why it happens
Depression usually develops where biological vulnerability meets prolonged strain: bereavement, unemployment, a controlling or violent relationship, chronic illness, isolation after migration or years of suppressed conflict at home. In our clients it is frequently masked as physical complaints, headaches, body aches, fatigue, because those are easier to say out loud than sadness.
When to speak to a professional
Speak to a clinician if low mood has lasted more than two weeks, if you can no longer meet your responsibilities, if you are withdrawing from everyone or at any point if you are having thoughts of ending your life, that last one is urgent, not something to observe for a while.
How we treat depression
Treatment works on two fronts at once: reactivating the parts of life that depression has shut down, and dismantling the self-critical thinking that keeps it in place.
Behavioural activation
A structured, gentle rebuild of daily activity, the intervention with the fastest measurable effect on low mood, and the one that restores energy rather than waiting for it.
Cognitive Behavioural Therapy (CBT)
Working on the automatic self-blame, hopelessness and ‘I am a burden’ beliefs that keep depression self-sustaining.
Interpersonal work
Addressing grief, role changes and family conflict directly, since these are the most common triggers we see in Pakistani clients.
Coordinated psychiatric care
For moderate to severe depression we can coordinate with a psychiatrist so medication and therapy are aligned. Therapy continues either way.
What a course of treatment looks like
Usually 10 to 16 weekly sessions, with a review at session six. If there is any risk to life we build a written safety plan in the first session and review it every time.
Start the check-indepression self-check
Over the last two weeks, how often has each been true? This is an educational screen, not a diagnosis, a clinician confirms anything it suggests.
1. Little interest or pleasure in doing things
2. Feeling down, hopeless or empty
3. Sleeping too little or too much
4. Feeling tired or having little energy
5. Feeling bad about yourself, or that you are a failure or a burden
